The Union Cabinet's decision on September 11, 2024, marks a momentous occasion for India's healthcare landscape. Under this significant expansion of the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB PM-JAY), all senior citizens aged 70 and above are now eligible for free health insurance coverage of up to ₹5 lakh per family, regardless of their financial standing. This move is set to bring immense relief and security to approximately 4.5 crore families, encompassing around 6 crore senior citizens across the nation.
This initiative underscores the government's steadfast commitment to ensuring health security for its elderly population, who often face significant healthcare costs. By removing the financial barrier to quality medical treatment, the expansion of AB PM-JAY aims to provide dignity and peace of mind to millions of senior citizens, allowing them to access necessary secondary and tertiary care hospitalization without the burden of out-of-pocket expenses.
The Foundations of Ayushman Bharat
Launched in 2018, Ayushman Bharat is celebrated as the world's largest government-funded health assurance scheme. Its primary objective is to make healthcare services accessible and affordable, aiming to achieve universal health coverage by 2030. The scheme's twin pillars are the establishment of Health and Wellness Centres (HWCs) to provide comprehensive primary healthcare and the Pradhan Mantri Jan Arogya Yojana (PM-JAY), offering secondary and tertiary care to vulnerable populations.
Initially, AB PM-JAY provided health cover of ₹5 lakh per family per year to approximately 55 crore beneficiaries, corresponding to 12.37 crore families in the economically vulnerable bottom 40% of India's population. Over the years, its reach has steadily grown, with many States and Union Territories converging their own health insurance schemes with AB PM-JAY to cover more than 18 crore families.
Unprecedented Expansion and Reach
Inclusion of Frontline Workers
Beyond the recent inclusion of senior citizens, the scheme saw another significant expansion earlier in 2024. In March 2024, approximately 37 lakh families of Accredited Social Health Activists (ASHAs), Anganwadi Workers (AWWs), and Anganwadi Helpers (AWHs) were also brought under the umbrella of AB PM-JAY. This acknowledges the invaluable contribution of these frontline workers to India's public health system and provides them with much-needed health security.
Current Beneficiary Numbers
The impact of Ayushman Bharat is clearly reflected in its growing numbers. As of November 30, 2024, approximately 36 crore beneficiaries have been verified under AB PM-JAY. Furthermore, the scheme has authorized 8.39 crore hospital admissions, amounting to services worth over ₹1.16 lakh crore. As of March 24, 2025, more than 36.9 crore Ayushman cards have been issued, facilitating seamless access to healthcare services for eligible individuals.
Impact on Out-of-Pocket Expenditure
One of the most critical successes of AB PM-JAY has been its role in reducing Out-of-Pocket Expenditure (OOPE) for beneficiaries. The Economic Survey 2024-25 highlights that the scheme has led to savings exceeding ₹1.25 lakh crore, significantly alleviating the financial burden on households. This reduction in OOPE is a testament to the scheme's effectiveness in providing social security and enhancing financial protection against catastrophic health expenses.
The government's increased focus on health spending is also evident. Between Fiscal Year 2015 and Fiscal Year 2022, Government Health Expenditure (GHE) rose significantly from 29.0% to 48.0%, while OOPE declined from 62.6% to 39.4%. This shift indicates a healthier financial ecosystem for healthcare in India, where a greater proportion of health spending is borne by the government rather than individuals.
Modi 3.0’s Broader Healthcare Vision
Prime Minister Modi's third term, often referred to as Modi 3.0, has outlined an ambitious vision for the healthcare sector. The overarching goal is to enhance the affordability, availability, and quality of healthcare services across the country. This includes substantial investments in healthcare infrastructure, particularly in rural and underserved regions, with a focus on building new hospitals, upgrading existing facilities, and improving medical technology.
Preventive care and public health initiatives are also high on the agenda. The government is emphasizing vaccination programs, combating communicable diseases, and promoting healthy lifestyles. The integration of 'Healthtech' or health technology, including telemedicine and AI, is seen as essential to address persistent challenges and make healthcare more inclusive and effective.
In the first 100 days of Modi 3.0, other significant healthcare initiatives have been rolled out or reinforced, such as the U-WIN portal for digitized immunizations and a special campaign to raise awareness about sickle cell disease. Efforts to create 75,000 more medical seats in the next five years also aim to strengthen the medical workforce.
Looking Ahead: Potential Enhancements
While the September 2024 expansion is a major stride, discussions continue regarding further enhancements to Ayushman Bharat. As of July 2024, the government was actively considering proposals to potentially double the beneficiary base under the scheme over the next three years. Furthermore, there were deliberations about increasing the annual insurance coverage limit from ₹5 lakh to ₹10 lakh.
These proposed changes, if approved and announced, are expected to significantly broaden the scheme's reach and provide even greater financial protection against high-cost treatments such as transplants and cancer care. Such expansions would entail an estimated additional expenditure of ₹12,076 crore annually for the exchequer. While these are still under consideration, they reflect a continuous effort to evolve and strengthen India's flagship health insurance program.
